Qualifications

Responsibilities

• Assess and discharge planning via multi-system assessments.
• Plan individualized nursing care and coordinate with hospital departments and community agencies.
• Implement care per physician plan and document in the EMR.
• Evaluate patient response to treatment and participate in unit quality assurance.
