Central Ohio Primary Care Physicians,Inc is seeking a Triage Specialist-RN (Day Shift) to perform telephonic nursing triage, document interactions in EPIC, and coordinate with the care team in Westerville, OH.

The role requires a Bachelor’s degree in Nursing, an active Ohio RN license, and at least 2 years of RN experience in hospital or medical practice settings, with telephonic triage experience preferred. Hybrid schedule with M-F hours.

Qualifications

• Bachelor’s degree in Nursing is required.
• Active and unrestricted registered nurse license in Ohio is required.
• Minimum of 2 years RN experience in hospital or medical practice setting is required.
• Experience in telephonic nursing triage is preferred.

Responsibilities

• Conduct telephone triage for patient health complaints using an electronic triage tool.
• Navigate multiple electronic platforms to assess health complaint, chart, scheduling, and care coordination.
• Document nursing triage notes in the EHR for clear documentation.
• Record phone call details in tracking system for data collection.
• Assess, identify, and close patient care gaps.
• Develop positive working relationships with COPC staff and physicians to deliver high-quality patient care.
