Job summary

Humana Inc. is seeking a Care Manager, Telephonic Behavioral Health 2 to assess and support members’ wellness through telephonic care coordination. Duties include evaluating needs, guiding to resources, and facilitating engagement with care plans.

Qualifications

  • Proficiency with Microsoft Office programs (Word, Excel, PowerPoint).

Job description

Become a part of our caring community The Care Manager, Telephonic Behavioral Health 2 , in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action. The Care Manager, Telephonic Behavioral Health 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures. Use your skills to make an impact.

Required Qualifications
  • Current valid social service license, e.g. LMSW, LCSW, LPC, LMHC
  • Master's degree in a behavioral-health field, such as social work, counseling, or related health discipline from an accredited university
  • 2+ years or more of experience working as a behavioral health professional
  • Experience with behavioral change, health promotion, and coaching
  • Proficiency with Microsoft Office Programs, e.g. Word, Excel & PowerPoint, shared systems, troubleshoot and resolve general technical issues
Preferred Qualifications
  • Experience supporting patients telephonically
  • Experience working in Community Mental Health or as part of a crisis response team or Assertive Community Treatment (ACT) team, and substance use disorder treatment
  • Experience working with both children and adults
  • Bilingual (English/Spanish) - Bilingual Language in both English and Spanish – Language Proficiency Assessment will be performed to test fluency in reading, writing and speaking in both languages.

The schedule for this remote telephonic position: Eight (8) hour shift between 8:00 AM and 6:00 PM in your local time zone. Occasional over time may be requested as business needs.

Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Work-At-Home Requirements

To ensure Home or Hybrid Home/Office associates' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office associates must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested Satellite, cellular and microwave connection can be used only if approved by leadership Associates who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense Humana will provide Home or Hybrid Home/Office associates with telephone equipment appropriate to meet the business requirements for their position/job Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information

To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Compensation

Scheduled Weekly Hours 40. The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $59,300 - $80,900 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
