Responsibilities

• Promotes patient and family centered care in a healing environment.

• Participates in patient safety, quality, and regulatory compliance activities.
• Develops and documents plans of care with interprofessional team and patient/family.
• Assesses physical, psychosocial, and emotional needs and communicates care plans across the continuum.
