Qualifications

• Nursing Diploma/Associate's required; BSN preferred.
• Minimum 2–5 years in Med/Surg or Telemetry RN.
• BSN degree considered a plus.

Responsibilities

• Identify and resolve gaps in care from hospitalization to ambulatory care.
• Lead development and implementation of complex care plans with the interprofessional team.
• Champion evidence-based practice and drive practice changes.
• Evaluate patient and family experience to improve quality of care.
• Maintain safety and readiness in a high-acuity unit.
