Qualifications

Responsibilities

• Assess physical, social, and psychological status and synthesize information from interdisciplinary team.
• Develop individualized plan of care with patient, family involvement.
• Coordinate and delegate care to the interdisciplinary team to execute the plan.
• Evaluate patient responses and revise the plan as needed.
• Uphold professionalism, legal/ethical standards, and maintain accurate documentation.
