• Complete patient assessments and identify needs across physical, emotional, psychosocial, educational, discharge and cultural aspects.
• Develop and implement individualized care plans with measurable goals.
• Coordinate with interdisciplinary teams and use equipment following policies and protocols.
• Reassess patients after interventions and update the care plan as needed.
• Recognize changes in patient condition and respond promptly.
