Job summary

Belle is redefining primary care for Medicare members through telehealth and preventive foot care. This physician role focuses on delivering telehealth visits, creating care plans, and coordinating with a team to manage chronic conditions for seniors.

You'll work entirely remote, provide E/M and Chronic Care Management, and help integrate care across nurses, NPs, and CHWs, while documenting thoroughly in our EHR and supporting preventive care initiatives.

Qualifications

• MD or DO with Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics.
• Active Georgia medical license (multi-state licensure a plus).
• Credentialed with major plans: UHC, Aetna, Humana.
• Strong interest in preventive care, senior health, or chronic condition management.

Responsibilities

• Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management).
• Create and manage personalized care plans for ongoing conditions.
• Collaborate with our integrated care team (nurse case managers, NPs, CHWs).
• Ensure thorough documentation in our EHR and compliance with quality measures.
• Engage patients in preventive care to reduce avoidable complications.

What We're Looking For

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Active medical license in Georgia— multi-state licensure a plus
• Credentialed with major plans: UHC, Aetna, Humana
• Strong interest in preventive care, senior health, or chronic condition management
• Tech-savvy and comfortable practicing via telehealth
• Collaborative, mission-driven mindset with excellent communication skills
