Job summary

Belle is redefining primary care for Medicare members through telehealth and coordinated care. This remote physician role supports in Medicare patients with E/M and Chronic Care Management, focusing on preventive care and optimized outcomes for seniors.

You’ll collaborate with an integrated care team, document thoroughly in our EHR, and help extend clinical reach while enjoying a balanced, mission-driven work culture. A Georgia license and multi-state credentials are a plus.

Qualifications

• MD or DO with board certification or eligibility in Family/Internal Medicine or Geriatrics.
• Active medical license in Georgia; multi-state license a plus.
• Credentialed with major plans (UHC, Aetna, Humana).
• Strong interest in preventive care and senior health; telehealth comfortable.
• Collaborative, mission-driven mindset with excellent communication.

Responsibilities

• Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management).
• Create and manage personalized care plans for ongoing conditions; collaborate with nurse case managers, NPs, CHWs.
• Ensure thorough documentation in our EHR and compliance with quality measures.
• Engage patients in preventive care to reduce avoidable complications.

What We’re Looking For

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Active medical license in Georgia— multi-state licensure a plus
• Credentialed with major plans: UHC, Aetna, Humana
• Strong interest in preventive care, senior health, or chronic condition management
• Tech-savvy and comfortable practicing via telehealth
