Job summary

Bakersfield Post Acute is seeking a compassionate Speech-Language Pathologist (SLP) for telehealth/per diem work. The role involves evaluating and treating residents with speech, language, cognitive-communication, and swallowing disorders, coordinating with a multidisciplinary team, and delivering care remotely via approved platforms.

The ideal candidate holds a CA SLP license, has strong clinical judgment, and telehealth experience.

Qualifications

• Master's or doctoral degree in Speech-Language Pathology from an accredited program.
• Current California SLP license in good standing.
• ASHA CCC preferred.
• Skilled nursing/long-term care experience preferred.
• Telehealth experience preferred.
• Proficiency with Net Health and PointClickCare preferred.

Responsibilities

• Conduct comprehensive evaluations of speech, language, cognitive-communication, voice, and swallowing needs.
• Develop and update individualized plans of care with measurable goals.
• Provide medically necessary treatment via telehealth platform.
• Monitor progress and modify treatment plans as needed.
• Educate residents, families, and caregivers on goals and strategies.
• Coordinate with physicians, nursing staff, and rehabilitation team.
