• Executes nursing process and coordinates care across the continuum.
• Coordinate care using health informatics and HT technologies to improve access.
• Triages data from HT patients and intervenes to prevent complications.
• Collaborates with HBPC, MH, SW, Pharmacy, and Nutrition teams.
• Delivers patient education and healthcare coaching for self-management.
• Supports interdisciplinary team with evidence-based practice.
