Job summary

The U.S. Department of Veterans Affairs in Wisconsin is seeking a Registered Nurse (RN) - Home Telehealth (HT) Care Coordinator to provide evidence-based care and coordinate telehealth initiatives for Veterans, including education and self-management support.

This is not a remote position. The role requires coordinating care for a panel of Veterans across the continuum, ensuring timely, high-quality, and cost-effective services while collaborating with primary care, specialists, and community

Responsibilities

• Providing competent, evidence-based nursing care to assigned patients.
• Coordinating care focused on patient education, self-management and satisfaction.
• Managing a panel of patients across the continuum to ensure timely, high-quality, cost-effective care.
• Facilitating shared decision making with the Veteran and developing patient-specific plans of care.
• Advocating for telehealth program needs and elevating issues to facility leadership.
• Coordinating communication across primary care, specialty care, other professionals, clinics and community agencies.
• Monitoring core quality and performance measures for telehealth programs.
• Developing mechanisms to evaluate quality, safety, efficiency, utilization, productivity and effectiveness of telehealth.
• Carrying out other duties as assigned.
