Job summary

Navitus Health Solutions seeks an RN Case Manager for Telehealth to improve members health through tele-pharmacy and outreach programs. You will support leadership with clinical resources, review medication reviews, and supervise technicians while ensuring adherence to HIPAA and regulatory standards.

The role offers remote engagement with members and prescribers, with potential location from St Louis, MO or Brentwood, TN, and a strong focus on program development and cost-effective formulary

Qualifications

• 2+ years clinical experience including direct consultation.
• Experience in a clinical call center is preferred.
• Experience as a member of a multi-disciplinary team is preferred.
• Knowledge of data privacy practices and HIPAA regulations is preferred.

Responsibilities

• Develop clinical algorithms, pathways and call scripts for pharmacy staff.
• Review and update algorithms and scripts yearly with updated clinical guidelines.
• Develop documentation standards for all clinical outreach.
• Collaborate with staff to maintain a high quality, consistent product.
• Facilitate communication and education to clients, providers and internal staff.
• Provide clinical outreach to members, prescribers and providers to enhance care coordination.
• Stay current with guidelines and integrate into programs and outreach.
