Job summary

CareMed is reshaping its telehealth program into a rotating clinical coverage model and launching CCM and BHI programs. The Director of Telehealth will own the program, build and manage the clinical team, redesign workflows, and oversee CCM/BHI operations across multiple states with after-hours coverage.

The role supervises NP/PA clinicians, ensures quality, and drives financial performance while coordinating with facilities and executive leadership.

Qualifications

Active NP or PA license in at least one CareMed operating state; multi-state licensure preferred.
Minimum 5 years clinical experience in primary/geriatrics/long-term care.
2–3 years in clinical leadership or program management overseeing clinicians.
Experience with telehealth service delivery in skilled nursing, assisted living, or similar settings.
Knowledge of CMS CCM/BHI billing and documentation requirements.

Responsibilities

Own end-to-end build-out and ongoing management of the telehealth program, including the clinical rotation schedule.
Supervise the clinician team: scheduling, coaching, performance management, and escalation support.
Liaise between telehealth team, facility partners, and executive leadership on program performance.
Own CCM/BHI program build: eligibility, enrollment workflows, and consent.
Monitor enrollment and encounter volume against revenue targets and adjust staffing as needed.
Ensure CMS and payer compliance in documentation and billing practices.
