Job summary

Devoted Health is seeking a Devoted Preventative Primary Care Provider (NP/PA) to conduct telehealth visits and occasional in-home visits. You’ll perform comprehensive assessments focused on prevention, chronic condition support, and health maintenance, collaborating with a multidisciplinary team to deliver member-centered care.

Ideal candidates have 3+ years outpatient experience in primary care or geriatrics, active NP/PA license with willingness to obtain additional licenses within 90 days,

Qualifications

• Active APRN/NP or PA license.
• 3+ years of outpatient clinical practice, ideally in primary care, family medicine, internal medicine, or geriatrics.
• Active and clear NP/PA license (willing to obtain additional licenses within 90 days)
• Active BLS certification at time of hire.
• Comfort with delivering care via video telehealth and using electronic documentation systems.

Responsibilities

• Conduct primarily telehealth video visits, with limited in-home visits.
• Perform comprehensive assessments and evidence-based preventive and chronic care.
• Identify and address care gaps, coordinate with care team, and plan whole-person care.
• Utilize technology and AI-enabled workflows to reduce admin burden and increase meaningful contact time.
• Communicate with empathy and clarity to build trust with members and caregivers.
