Applicants should hold an LPN license and have experience in primary care/geriatrics or skilled nursing. Flexibility to travel to multiple locations and collaborate with staff is required.

Qualifications

• Licensed Practical Nurse (LPN) with current license.
• Experience in primary care, geriatrics, or skilled nursing preferred.
• Must be flexible to travel across the market area and schedule needs.

Responsibilities

• Facilitate telehealth visits by setting up device connections.
• Remain present during visits to assist with communication and needs.
• Document vital signs and relay information to providers.
• Ensure equipment functions properly before appointments.
• Protect patient privacy and HIPAA compliance.
• Coordinate care with facility staff and team members.
• Assist with light administrative tasks to support care operations.
