Job summary

Vitability Health Of New Jersey is seeking a Medical Doctor to join our telehealth team, delivering high-quality virtual care to patients transitioning from hospital or rehabilitation settings to home. The ideal candidate will hold an MD or DO, have an active New Jersey license plus licensure in another state, and excel in remote patient care, communication, and care coordination with PAs/NPs and nurses.

Qualifications

• MD or DO degree required.
• Active NJ medical license and licensure in at least one other state.
• Strong communication and interpersonal skills for virtual care.

Responsibilities

• Provide telehealth consultations delivering comprehensive medical care.
• Collaborate with PAs, NPs, care navigators, nurses and the healthcare team on patient care plans.
• Address chronic conditions, medication management and preventive care to reduce readmissions.
• Ensure compliance with Medicare, Medicaid and other insurance requirements for telehealth.
• Document patient interactions in EMR in HIPAA-compliant manner.
