Vitability Health Of New Jersey is seeking a skilled Medical Doctor to join our telehealth team, delivering virtual care to patients transitioning from hospital or rehabilitation settings to home. A NJ-focused multi-state license is required to practice across states.

The role offers fully remote telehealth, a collaborative environment with PAs, NPs, nurses and care navigators, and a strong emphasis on high-quality patient outcomes through innovative virtual healthcare solutions.

Qualifications

• MD or DO required and board eligibility recommended.
• Active medical license in New Jersey and at least one other state (multi-state licensure).
• Strong communication and interpersonal skills for virtual care.

Responsabilités

• Provide telehealth consultations and ongoing medical care.
• Collaborate with PAs, NPs, care navigators, nurses to create care plans.
• Address chronic conditions, medications, and preventative care to reduce readmissions.
• Ensure compliance with Medicare, Medicaid and other insurance requirements for telehealth.
• Document patient interactions in EMR per HIPAA guidelines.
