The Hospital for Behavioral Medicine in Worcester, MA is seeking a Tele-Assessment Clinician to support referrals, assess patients of all ages with a physician, and assist with discharge planning in a per diem, on-call capacity.

This role focuses on providing resources, timely intake processing, and effective care coordination for the community, including documentation and follow-up as needed.

Qualificações

• Education: Registered Nurse or Master’s Degree in a human relations field required.
• Massachusetts licensure as a mental health professional preferred.
• Two+ years’ intake assessment experience in an acute psychiatric setting; customer contact.
• CPR certification and CPI training within 30 days of employment before restraining procedures.

Responsabilidades

• Inquiry calls are documented and processed according to Intake policies.
• Patients are assessed and stabilized prior to leaving hospital or admission to hospital.
• On-call doctor is notified and assisted with patient interview.
• Admission paperwork is completed per hospital policy.
• EMTALA log is completed with all information documented.
• Follow-up calls are made to those failing to show up for appointments.
• Pre-certification is obtained in accordance with insurance requirements.
• Medication is obtained after hours as ordered with proper documentation.

Your Work Matters How will you make a difference?

The Tele-Assessment Clinician is responsible for assisting in the process of responding to referrals and other requests for services and information. The Tele-Assessment Clinician assesses all ages and collaborates with a physician to determine a proper disposition. The Tele-Assessment Clinician provides technical assistance to the discharge planning process, under the direction of the Director of Intake. Tele-Assessment Clinician is often the first contact with the community and therefore excels at providing resources, and support to callers and walk-ins. Tele-Assessment Clinician provides general support to the case management program.

• Inquiry calls are documented and processed in accordance with Intake policies.
• Patients are assessed and stabilized prior to leaving hospital or admission to hospital arranged.
• On call doctor is notified and assisted with patient interview.
• Admission paperwork is completed in accordance with hospital policy.
• Intake (EMTALA) log is completed with all information documented.
• Follow up calls are made to those failing to show up for appointments.
• Pre-certification is obtained in accordance with insurance requirements.
• Medication is obtained after hours as ordered by the physician with proper documentation of drugs removed and accountability maintained.

Your Experience Matters What we’re looking for:

• Education: Registered Nurse or Master’s Degree in a human relations field required.
• Licensure: Licensed to practice in the State of Massachusetts as a mental health professional is preferred.
• Experience: Preferably two or more years’ experience in intake assessment activities within an acute psychiatric setting. A minimum of two (2) years’ experience in a psychiatric setting including customer contact.
• Additional Skill Requirements: A working knowledge of the latest DSM and Mental Status Exam. A working knowledge of the Addiction and Recovery field. Successful completion CPR certification and CPI training to within 30 days of employment prior to assisting with restraining procedures.

Position Requirement:

• Must complete two week in-person orientation.

Compensation:
