Vasona Creek Healthcare Center in Los Gatos/South Bay is seeking a Physical Therapist to join a high-tech, collaborative rehabilitation team.

You will provide comprehensive PT services in a short-term rehab setting, with advanced modalities and a focus on outcomes; licensed PT needed in CA; opportunities include full-time, part-time and PRN, with immigration sponsorship and relocation bonus options.

Qualifications

• Current California Physical Therapist license required.
• Experience in skilled nursing, post-acute, or rehabilitation settings preferred.
• Strong clinical reasoning and communication skills.
• Passion for rehabilitation and helping patients achieve meaningful functional outcomes.
• Commitment to evidence-based, patient-centered care.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized, evidence-based treatment plans.
• Provide skilled therapy services to short-term rehabilitation and long-term care residents.
• Treat patients recovering from joint replacements, cardiac events, strokes, and other medical conditions.
• Utilize specialized rehabilitation programs, including Acute Neuro Rehabilitation and Pain Management.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with PTAs, OTs, SLPs, nursing, and other members of the interdisciplinary team.
• Educate patients and families regarding treatment plans, exercises, mobility, and strategies for continued progress.
• Promote safe mobility, functional independence, and successful discharge planning.
• Maintain accurate and timely clinical documentation.
• Advocate for patients and support their individual rehabilitation goals.
