Job summary

Community Health and Wellness Partners is seeking a Team Physician in Urbana to provide primary care within a team-based model. You will lead a clinical team, oversee patient care operations, and ensure high-quality, whole-person care in a federally qualified center.

The role requires an MD or DO, board certification in the specialty, Ohio licensure, and proficiency with EHR systems. Expect a full-time schedule with occasional after-hours as needed.

Qualifications

• Graduate of an ACGME accredited residency program in the specialty area.
• Board certification within specialty (State Medical Board of Ohio).
• Licensure: Medical Doctor (MD) or Doctor of Osteopathic (DO) with valid license to practice medicine.

Responsibilities

• Direct clinical care within primary care scope as Team Physician.
• Lead and coordinate a team-based care model to meet care quality and productivity goals.
• Ensure accurate documentation, charting, and confidentiality of patient information.
• Supervise and coordinate activities of patient care and team care.
• Maintain HIPAA compliance and participate in quality improvement initiatives.
