Job summary

Crouse Health in Syracuse, NY seeks a Care Coordination Team Leader to oversee the department, guide daily workflow, coordinate staff scheduling, and mentor new hires. This full-time role requires NY RN licensure and 3–5 acute hospital experience, with a BSN preferred.

Salary ranges from $88,000 to $95,000 annually. Schedule is full-time, Monday–Friday, 8:00am–4:30pm, with 6 weekends per year. Benefits include Medical, Dental, Vision, FSA, 401K, tuition assistance, EAP, and employee discounts.

Qualifications

• Must be licensed as a Registered Nurse in New York State.
• Three to five acute hospital experience required.
• Excellent oral and written communication and interpersonal skills.
• Knowledge of federal and state regulations (DOH, Medicare/Medicare).
• Strong organizational and problem solving skills.

Responsibilities

• Acts as a lead point-of-contact and knowledge expert to all employees within department or assigned area.
• Effectively coordinates staff scheduling, workflow, and delegation of daily tasks to ensure work is completed on time.
• Notifies department leadership when there are issues with daily workflow and performance.
• Identifies and resolves issues affecting the completion of delegated tasks and duties.
• Provides on-going training to existing team members as required.
• Acts as a resource during on-boarding and training of new employees.

Pay Range

$88,000.00 - $95,000.00 / annual

Schedule

Full-Time, Monday-Friday, 8:00am-4:30pm. Required to work 6 weekends per year.

Care Coordination Team Leader Responsibilities

• Acts as a lead point-of-contact and knowledge expert to all employees within department or assigned area.

• Notifies department leadership when there are issues with daily workflow and performance.
• Identifies and resolves issues affecting the successful completion of delegated tasks and duties.
• Provides on-going training to existing team members as required.
• Acts as a resource during the on-boarding and training of new employees.
• Provides support, guidance, leadership and motivation to promote maximum performance.
• Assume other duties as assigned by department leadership.

Care Coordination Team Leader Requirements

• Must be licensed as a Registered Nurse in New York State.
• Associate’s Degree required, Bachelor’s Degree (BSN) preferred.
  • Bachelor’s Degree required within 8 years of being hired for those licensed after 2017.
• Three (3) to Five (5) acute hospital experience required.
• Excellent oral and written communication and interpersonal skills.
• Knowledge of federal and state regulations (DOH, Medicare/Medicare).
• Strong organizational and problem solving skills.
• Knowledge of third party payers and/or managed care principles.
