Job summary

Oaklawn Psychiatric Center in South Bend, Indiana, is seeking a Team Leader - Case Management to provide clinical oversight for a team of case managers and peer support professionals within the Permanent Supportive Housing division. This role emphasizes teamwork, accountability, and community collaboration.

You will hire and train staff, monitor productivity and documentation, participate in multi-department meetings, and engage with partners while ensuring quality client care.

Qualifications

• Bachelor's degree required; Human or social services preferred.
• Previous supervisory experience preferred.
• Must provide diploma and official transcripts
• Valid driver's license and proof of insurance

Responsibilities

• Provide clinical oversight and support to a team of case managers and peer support professionals in the Permanent Supportive Housing division.
• Hire and train new team members, monitor productivity, clinical documentation, and budgets.
• Attend regularly scheduled meetings with multi-departmental leaders.
• Maintain relationships with community partners and organizations.
• Conduct biopsychosocial assessments and make recommendations for treatment.
• Collaborate with a multidisciplinary team including client, family, referral sources, and other Oaklawn providers.
• Provide direct care to clients as needed.
• Monitor service delivery for quality assurance and client satisfaction.
• Provide professional development training.
• Evening hours are expected 1x/month

What You’ll Do as a Team Leader:

Primary responsibilities:

• Provide clinical oversight and support to a team of case managers and peer support professionals in the Permanent Supportive Housing division.
• Build and maintain a cohesive team, hiring and training of new team members, monitoring team productivity, clinical documentation, departmental budgets, and other supervisory duties as assigned
• Attend regularly scheduled meetings with multi-departmental leaders to share organizational and team updates
• Maintain relationships with community partners and organizations
• Conduct comprehensive biopsychosocial assessments and make recommendations for treatment
• Work collaboratively with a multidisciplinary team that includes the client, family, referral sources, and other providers within Oaklawn
• Provide direct care to clients as needed
• Provide routine monitoring of case management service delivery for quality assurance and client satisfaction
• Professional development training
• Evening hours are expected 1x/month

Minimum Hiring Requirements for a Team Leader:

• Bachelor's degree required; Human or social services preferred.
• Previous supervisory experience preferred.
