You will supervise MSWs and case managers, conduct psychosocial assessments, provide counseling, and coordinate with health providers, while supporting staff training and program development.

Qualifications

• Requires Licensed Master Social Worker (LMSW).
• Experience with mental illness, substance abuse, and homeless adults.

Responsibilities

• Supervise MSWs and case managers; oversee caseloads.
• Conduct psycho-social assessments for assigned clients.
• Provide individual counseling within LMSW scope.
• Deliver emergency and crisis interventions.
• Provide ADL skills training and supportive counseling.
• Document accurately per DHS and funder requirements.
• Participate in interdisciplinary team meetings and program development.
• Establish advocacy with community resources.
• Support training and onboarding of staff.
• Link clients to substance abuse and mental health services.
• Coordinate treatment plans with Nurse Practitioner or community provider.
• Lead three weekly group sessions and report to Program Director.
