Job summary

RiseMe is seeking a Team Lead of START Care Coordinators to provide administrative supervision and clinical triage support, ensuring rapid crisis response and coordinated care for individuals served through START.

The role requires a Master’s degree in a human services field, at least two years of relevant experience, one year of supervision, and commitment to becoming a certified START Coordinator within 12 months.

Qualifications

Responsibilities

• Provides daily clinical triage support to START Coordinators.
• Shares on-call responsibilities to support 24/7 crisis response with 1-hour telephonic and 24-hour in-person response.
• Maintains a small active caseload of approximately five individuals until START Coordinator certification is achieved.
• After certification, maintains a fluid and temporary caseload to support START Coordinators when needed.
• Provides administrative supervision to START Coordinators.
• Maintains effective linkages and relationships with community partners.
• Participates in recurring meetings with START leadership, the clinical team, and the resource center.
• Ensures coordination of support meetings and crisis plans for individuals served through START.
• Supports timely review and response to crisis situations, service needs, and changes affecting individuals served.
• Monitors assigned and temporary caseload responsibilities to promote continuity and appropriate follow-through.
• Supports person-centered crisis planning and service coordination for individuals with intellectual and developmental disabilities and mental health needs.
• Collaborates across systems to strengthen supports and reduce barriers during crisis situations.
• Provides direct service and caseload support when needed to maintain responsive START services.
• Develops cross-system crisis plans, intake and assessment documentation, intervention and outcome documentation, and other applicable records of services provided.
• Maintains required START certification and completes applicable training and program requirements.
• Completes documentation accurately and timely and follows agency, program, confidentiality, and recordkeeping requirements.
• Performs other duties as assigned by the supervisor.

FLSA CLASSIFICATION: Non-Exempt POSITIONS SUPERVISED: START Coordinators

POSITION OVERVIEW:

The Team Leader provides administrative supervision and daily clinical triage support to START Coordinators. The position supports timely crisis response, cross-system planning, coordination of support meetings and crisis plans, documentation of services, community partnerships, and collaboration with START leadership, the clinical team, and the resource center.

ESSENTIAL POSITION RESPONSIBILITIES:

Service Delivery

• Provides daily clinical triage support to START Coordinators.
• Shares on-call responsibilities to support 24/7 crisis response, including a telephonic response within one hour and an in-person response within 24 hours of the call.
• Maintains a small active caseload of approximately five individuals until START Coordinator certification is achieved.
• After certification, maintains a fluid and temporary caseload to support START Coordinators when needed.

Care Coordination & Collaboration

• Provides administrative supervision to START Coordinators.
• Maintains effective linkages and relationships with community partners.
• Participates in recurring meetings with START leadership, the clinical team, and the resource center.

POSITION REQUIREMENTS:

• Master's degree in social work, counseling, psychology, or a human services field.
• At least two (2) years of experience providing services to people with intellectual and developmental disabilities and mental health needs.
• At least one (1) year of supervisory experience.
• Must become a certified START Coordinator within twelve (12) months of employment as Clinical Team Leader.
• Prior experience as a START Coordinator preferred.

Driver Level:

Level 2 - Routine driving for work, errands, or transporting others; may be infrequent or on an irregular basis.

Position Flexibility:

Onsite (No Remote Work Definition A) - This is an onsite position with no regular remote work option. Limited flexibility for remote work may be granted in the event of office closures due to inclement weather; otherwise, remote work is not available.
