Heritage Behavioral Health Center, Inc. is seeking a proactive Team Lead for Integrated Housing Services to provide leadership across four residential settings.

You will supervise a multidisciplinary team, oversee IATP development, and ensure high-quality, trauma-informed care for individuals with complex mental health needs and co-occurring disorders. Responsibilities include coordinating services across locations, collaborating with clinicians and external providers, and promoting housing

Qualifications

• Master’s Degree in Social Work, Psychology, Counseling, or related field is required.
• Must be eligible for licensure as LCPC or LCSW in Illinois.
• Valid driver’s license and reliable transportation required.
• Experience supervising residential or community-based behavioral health programs preferred.

Responsibilities

• Provide direct supervision, guidance, and support to the team of community support and recovery staff.
• Facilitate regular team meetings, case consultations, and supervision to ensure quality service delivery.
• Support staff development through coaching, training, and performance feedback.
• Oversee the development and implementation of Individualized Assessment and Treatment Plans (IATPs).
• Deliver and support mental health and substance use treatment services in residential settings.
• Ensure services are trauma-informed and recovery-oriented.
• Coordinate services across residential locations to ensure consistent implementation of programs.
• Collaborate with outpatient providers, medical staff, and internal programs for continuity of care.
• Promote housing stability and community integration.
• Ensure documentation and program operations meet agency policies and regulatory standards.
• Foster collaboration among residential programs, clinical teams, and agency leadership.

Your Role:

Team Lead, Integrated Housing Services

Heritage Behavioral Health Center is seeking a dynamic and mission-driven Team Lead, Integrated Housing Services to provide leadership, supervision, and oversight of community support and recovery support services across four residential settings.

This leadership role is responsible for overseeing supervisors and multidisciplinary teams that provide comprehensive, person-centered services to individuals living with severe and persistent mental illness and co-occurring substance use disorders. Services are tailored to meet each person’s unique needs and include mental health counseling, skill building, medication management, budgeting support, healthcare coordination, and linkage to community resources.

The role promotes a recovery-oriented approach that supports individuals with mental health and co-occurring substance use disorders in living in the least restrictive environment possible. The Team Lead coordinates care across team members, ensures service quality and compliance with agency and regulatory standards, and fosters collaboration to address residents’ clinical, recovery, and daily living needs.

Team Leadership and Supervision

• Provide direct supervision, guidance, and support to the team of community support and recovery support team members.
• Facilitate regular team meetings, case consultations, and supervision to ensure quality service delivery.
• Support staff development through coaching, training, and performance feedback.

Clinical Oversight and Service Delivery

• Oversee the development and implementation of Individualized Assessment and Treatment Plans (IATPs) for clients.
• Provide and support delivery of individual and group mental health and substance use treatment services within residential settings.
• Ensure services are trauma-informed, recovery-oriented, and responsive to the needs of individuals with co-occurring disorders.

Program Coordination

• Coordinate services across residential locations to ensure consistent implementation of community support and recovery services.
• Collaborate with outpatient providers, medical staff, and other internal programs to ensure continuity of care.
• Support transitions into and out of residential services and promote successful community integration.

Housing Stability and Recovery Support

• Promote housing stability by ensuring residents receive appropriate life-skills support, recovery services, and clinical care.
• Assist staff in addressing barriers related to mental health, substance use, daily living, and community functioning.
• Encourage resident engagement in treatment, recovery programming, and community activities.

Quality and Compliance

• Ensure documentation, service delivery, and program operations meet agency policies and regulatory standards.
• Monitor service quality, productivity, and outcomes related to residential and recovery support services.
• Support continuous quality improvement efforts within residential programs.

Collaboration and Communication

• Serve as a liaison between residential programs, clinical teams, and agency leadership.
• Promote strong communication among staff supporting residents across locations.
• Participate in interdisciplinary meetings to coordinate care and address resident needs.

Knowledge, Skills, and Abilities:

• Demonstrated ability to work effectively with individuals with mental health and/or substance use diagnoses, including co-occurring disorders.
• Strong knowledge of community resources and social service systems, with the ability to research and connect clients to appropriate services.
• Knowledge of case management techniques and healthcare systems (psychiatric, substance use, medical, and related services).
• Ability to meet individuals where they are in their recovery in a non-judgmental and compassionate manner.
• Strong leadership and team collaboration skills.
• Ability to navigate ambiguity, uncertainty, and organizational change.
• Proficiency in electronic health records and documentation systems.
• Ability to think clearly and logically in high-risk, emotional, or potentially dangerous situations.
• Ability to provide timely and responsive communication to supervisors, staff, clients, and partners.

Qualifications

• Must be eligible for licensure as an LCPC or LCSW in Illinois.
• Valid driver’s license, reliable transportation, and proof of automobile insurance required.
• Experience supervising residential or community-based behavioral health programs preferred.
