Health Solutions is seeking a Housing Team Lead for the Adult Outpatient Services in Pueblo, CO. The role supports clients in accessing housing resources, maintains caseloads, and ensures HUD/DOH requirements are met. On-site, Mon–Fri, 8:00 AM–5:00 PM.

Responsibilities include team coordination, HMIS trainings, records management, and policy development. A Bachelor’s degree and CO driver license are required, with experience in housing vouchers preferred.

We’re looking for a Team Lead in Housing

to join us at our Adult Outpatient Services at 1304 Chinook Lane in Pueblo. Monday-Friday 8:00 AM – 5:00 PM.


What You’d Be Doing

RESPONSIBILITIES:


As a Housing Team Lead with the Housing Program, you would play a central role in helping our clients access services and resources and take the next step to wellness. The Housing Team Lead maintains their own caseload and is able to complete HUD/DOH requirements for housing vouchers. In addition, this position assists with outreach to individuals when needed. This position is required to provide services in client homes, in conjunction with services at Health Solutions, and at other locations in the community that provide a setting of natural support for the client. Duties include case management services, behavioral health counseling and outreach, EHR documentation, and documentation for HUD/DOH requirements, under the administrative supervision of a program supervisor.


Specifically, the Housing Team Lead is responsible for:


  • Conduct team meetings.

  • Communicate effectively with team members in order to assign tasks and ensure follow through occurs.

  • Provide oversight of HMIS duties and trainings to include VI-SPDAT trainings and recertifications

  • Maintain timely records for Housing Quality Inspections

  • Provide weekly or bi-weekly supervision for Housing staff.

  • Provide support and back-up as needed to Voucher Administrator

  • Keeping accurate, complete and timely records, including electronic records.

  • Complete Records reviews and of physical files for accuracy.

  • Complete DOLA audits as required.

  • Participation in staffing as needed.

  • Developing and maintaining policies, SOPs, and workflows.

  • Participate in community based committees to address homelessness needs

  • Property Management for 9 Unit Complex

  • Perform other job-related duties assigned by the Director or his/her designee.


Physical Requirements:


  • Requires standing, sitting, driving, and walking for prolonged periods of time

  • Occasionally lifts and carries items up to 20 pounds

  • Requires working under stress in emergency situations

  • Requires normal range of hearing and eyesight to record data electronically and communicate with clients

  • Occasional exposure to communicable diseases and bodily fluids

  • Requires occasional driving in adverse weather conditions

  • Requires frequent use of hands and fingers to operate a computer keyboard and mouse

Qualifications


  • Bachelor's degree in social work, psychology, or closely related field.

  • Experience working with housing vouchers, recertification, and state requirements.

  • Valid Colorado driver license, good driving record, and access to readily available transportation during working hours.

  • Computer proficiency.
