Overview

Barnes-Jewish St. Peters Hospital serves families in St. Charles, Lincoln, and Warren counties. It is a 102-bed community hospital offering inpatient and outpatient services, including the Siteman Cancer Center, Breast Health and Women’s Center, Cardiology Center, Neurosurgery Center, and Outpatient Surgery and Endoscopy Center.

The hospital is an Accredited Chest Pain Center with a Level II stroke certification and hosts various specialty clinics and outpatient services.

Role Purpose

Under the manager's direction, plans, directs, and coordinates unit activities on a designated shift. Performs direct patient care as needed and serves as a clinical resource for nursing and support staff. Assesses, plans, evaluates, and documents the nursing process for the assigned unit.

Responsibilities

1. Facilitates identification and resolution of care gaps from hospitalization to discharge and into outpatient settings. Critically assesses patient data to proactively address changes in condition and consults with the interprofessional team as appropriate.
2. Develops and implements complex individual care plans with defined goals in collaboration with the interprofessional team, patients, families, and caregivers, following established standards of care. The plan reflects expert clinical knowledge, anticipates barriers, incorporates team and family input, and seeks to improve patient outcomes.
3. Champions evidence-based practices through scientific inquiry, participates in journal clubs, facilitates practice changes, and supports ongoing education and practice improvement initiatives. Explores opportunities to advance nursing practice and community engagement.
4. Analyzes practice standards gaps, provides feedback to peers, and collaborates with leadership to enhance safe patient care. Evaluates patient and family experience to improve satisfaction and quality of care.
5. This is a safety-sensitive position requiring constant alertness and safe working practices.
