Job summary

Forsyth County Government is seeking an on-site Senior Services Activities Team Leader in Cumming, GA. This role leads recreational, educational, fitness, and wellness activities for older adults, oversees program implementation from start to finish, and supports daily center operations.

The position requires coordinating meals delivery, transportation, and client assessments, while supervising volunteers and maintaining program records. Certification to lead group fitness classes is preferred.

Qualifications

  • High school diploma or GED; supplemented by one (1) year previous experience providing programs and services to senior, with supplemental training in the area of assignment; or any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities for this job.
  • Must possess and maintain valid CPR/First Aid certification.
  • Some positions require a valid Georgia driver’s license.
  • Assignment to the Senior Services grant programs of Meals on Wheels, Congregate, and Respite requires passing a federal fingerprint background check.

Responsibilities

  • Coordinates activities of an assigned program/service designed to assist senior services clients with various aspects of life: assists the Senior Services Coordinator with program planning; provides assistance and guidance to team members regarding policies and completion of assigned tasks.
  • Responsible for the onsite distribution and home delivery of meals for seniors: places daily orders for meals to the contract provider; receives meals from contract provider, serves meals onsite, and boxes meals for home distribution; locates referrals for potential clients to receive homebound meal services; obtains and records directions to client homes; explains program and local guidelines prior to enrollment; completes referral forms, registration forms, and nutritional screening instrument; obtains copies of prescriptions from diabetic clients and places in client files; provides backup coverage for food preparation, meal delivery, client transportation, or other tasks.
  • Responsible for the Alzheimer’s patient assistance program for seniors: prepare the calendar of program and activities for the day program; provides structured program including activities such as memory stimulation, trivia questions, games, puzzles, crafts, singing, exercise, lunch, and snacks; works to provide clients with a relaxed atmosphere and a sense of security in surroundings and with volunteers; encourages clients to participate in activities; stimulates memory of personal life, birthdays, and happy times; coordinates short expeditions such as town tours, picnics, lake visits, or trips to ice cream store; and carries clients to restrooms as needed.
  • Participates in the assessment and registration of new and existing clients: schedules assessments; interviews participants to determine program eligibility, assess level of functioning, and identify interests; registers participants for programs as appropriate; conducts environmental reviews; compiles reports and maintains all related documentation; and coordinates admission and discharge procedures for all program participants.

Position Overview

The purpose of this classification is to serve as a lead worker in one of six functional areas of the senior services: meals, transportation, reporting and documentation, program resources and development, wellness and reassessments, and Alzheimer respite care. Individuals in this classification are expected to ensure that the tasks pertaining to each functional area are carried out correctly and efficiently; each team leader works under direct supervision.

The Senior Services Activities Team Leader is responsible for planning, executing, and leading engaging recreational, educational, fitness, and wellness activities for older adults. This position oversees program implementation from start to finish, including researching new activities, scheduling various presentations, leading fitness classes, and executing special events. The role also includes organizing activity spaces and storage areas, maintaining program supplies, and completing monthly reports to support program tracking and continuous improvement. The Activities Team Leader plays a key role in creating an inclusive, safe, and energetic environment that promotes active living, social connection, and overall well‑being for older adults, while providing energetic on‑site leadership during programs and events and supporting daily center operations. This position also supports program entry and support of Senior Services Activity Registration system. Certification to lead Group Fitness Classes preferred.

Essential Functions

Participates in the compilation and preparation of all reporting and documentation requirements for senior services and related programs: ensures compliance with all reporting mechanisms required by grants and or state agencies in order to secure funding for the programs; maintains detailed records of all programs and activities; and performs related administrative tasks.

Participates in the transportation program: coordinates the pick up and drop off of seniors participating in the day program and/or other activities provided by the department; assists with the scheduling of activities requiring transportation and ensures that drivers are available; documents daily trip totals and related activities; schedules maintenance of vehicles; operates a passenger van or small bus to transport clients or other individuals as needed; and ensures that drivers are following proper procedures and guidelines with regards to the transportation program, licensing, and its participants.

Participates in or provides programs to senior services clients: facilitates structured programs including activities such as educational programs, outings, memory stimulation, trivia questions, games, puzzles, arts and crafts, singing, exercise, lunch, and snacks; works to provide clients with a relaxed atmosphere and a sense of security in surroundings and with volunteers; encourages clients to participate in activities and transports clients to and from programs as necessary.

Interacts with and assists program participants as needed, to include activities of daily living: addresses concerns for participants and assists with problem resolution; maintains an awareness of the general well‑being of participants; reports any suspicion of problems that might require intervention of Adult Protective Services.

Prepares or completes various forms, reports, correspondence, client reports, waiting lists, referral forms, registration forms, nutritional screening instrument forms, calendars, mileage reports, reimbursement reports, or other documents.

Receives various forms, reports, correspondence, referrals, maps, medical resources, product instructions, policies, procedures, manuals, directories, reference materials, or other documentation; reviews, completes, processes, forwards or retains as appropriate.

Operates a computer to enter, retrieve, review or modify data; verifies accuracy of entered data and makes corrections; utilizes word processing or other computer programs.

Communicates with supervisor, employees, students, volunteers, other departments, clients, passengers, the public, community agencies, outside agencies, and other individuals as needed to coordinate work activities, review status of work, exchange information, or resolve problems.

Provides articles or information for the department newsletter, webpage, and other communications.

Additional Functions

Assignment to Sexton Hall may include the following additional duties: planning and overseeing programming, working with instructors in the areas of art, technology, fitness, etc. and overseeing classrooms, including a ceramics kiln, computer lab, and game room. Setting schedules, creating class materials, and managing registration. Assisting with the planning and implementation of special events such as concerts, dances, and game nights. Leads special projects such as the community garden. Assists with rentals at the facility, meeting with prospective renters, overseeing paperwork and staffing. Sexton Hall is an active adult life enrichment center for those age 50 and older.

Additional Functions: This classification is designated as a safety sensitive position which is subject to random drug and alcohol testing and any other drug testing methods as stated in policies adopted by Forsyth County concerning drug and alcohol. Performs general/clerical tasks, which may include answering telephone calls, making copies, sending/receiving faxes, filing documentation, picking up or delivering mail, or conducting visitor tours of senior center.

Personal Leave Accrual: 0‑4 Years: 20 days per year (6.16 hours per pay period); 5‑14 Years: 25 days per year (7.70 hours per pay period); 15+ Years: 30 days per year (9.24 hours per pay period).

Dental Insurance: Two dental plans from Delta Dental: Base $1,000 Plan; Buy‑Up $1,500 Plan.

Vision Insurance: Administered by Aetna and will offer office visit copays for exams, as well as copays for lenses, frames, and contacts.

Basic Life & AD&D Insurance: Term life insurance and accidental death and dismemberment (AD&D) protection for each full‑time employee; County‑paid benefit.

Disability Insurance – Short Term and Long Term: 100% paid by Forsyth County; Eligibility begins date of hire. Short Term Disability – 60% of weekly earnings to a maximum of $1,000.00 a week; eligibility begins 14 days after an accident or sickness and has a benefit period of 24 weeks. Long Term Disability – 60% of monthly earnings to a maximum of $5,000.00 a month; eligibility begins at 26 weeks to coordinate with the end of Short Term benefits and continues 24 months (Own Occupation).

Additional Life Insurance and AD&D: Rates vary based on amount of coverage selected; additional life insurance may be purchased for employee, spouse, and children.

Retirement Plan (401K): Fully vested at five years of employment – 20% graduated vesting annually. Eligible to begin contributions immediately upon hire. First 6 months through 1 year: County matches contributions dollar for dollar up to 5% of pay. Years 1‑5: County “seeds” your account with 5% of your pay each year, and matches 100% of what you contribute, up to 5% of pay. Years 5‑10: County continues its automatic “seed” contribution of 5% of pay, matches up to the first 5% of pay that you contribute, then adds a 50% match up to the next 5%. After 10 years of service: County continues to receive 100% match on the first 5% of pay you contribute and 50% on the next 5%; automatic contribution increases to 7.5% of your pay.
