Job summary

United Cerebral Palsy of Georgia is seeking a Team Lead to guide a dedicated team at a Newborn, GA site. You will oversee daily operations, assign tasks, monitor performance, and ensure a positive, productive work environment while maintaining safety standards.

Responsibilities include coaching staff, reporting to management, coordinating transportation and activities for residents, documenting progress, and ensuring adherence to care plans and policies.

Responsibilities

  • Inspire and motivate team to achieve performance goals.
  • Establish clear objectives and deadlines for the team and individuals.
  • Identify and resolve issues within the team or with projects.

  • Track team and individual performance and provide feedback.

  • Document using Therap system and other electronic systems.
  • Ensure goals align with each individual ISP and conduct evacuation drills.
  • Assist in planning person-centered outings and track health-related information.
  • Follow laws, policies and complete required training via Relias.

Job Summary:

A Team Lead, or Team Leader, is a professional who provides guidance, direction, and leadership to a team of employees, ensuring they achieve organizational goals and objectives. They are responsible for overseeing daily operations of a site-specific home(s), delegating tasks, setting deadlines, and fostering a positive and productive work environment. Team leads also play a crucial role in communication, problem-solving, and performance management within the team.

Duties/Responsibilities:
  • Inspire and motivate their team members to achieve performance goals and maintain a positive work atmosphere.
  • Establish clear objectives and deadlines for the team and individual members.
  • Identify and resolve issues that may arise within the team or with projects.
  • Required to report on team performance and progress to higher management.
  • Responsible for cultivating positive relationships with guardians as well as the greater community
  • Team leads track team and individual performance, providing feedback and support to improve outcomes.
  • Provides transportation and supervision for individuals as required to meet employment, recreational and leisure activities.
  • Team leads may be responsible for managing resources allocated to the team, such as budget, tools, or personnel.
  • Building positive relationships and fostering collaboration within the team is important.
  • Performs inspections of vans and ensures that all training for the safe transport of residents is followed including performing tie downs, seat belt usage.
  • Performs housekeeping responsibilities to include but not limited to, kitchens, bathrooms, windows, bedrooms, and floors.
  • Maintains all standards when preparing meals and managing diets. Follows guidelines for safe handling of food as well as ensures adequate supplies. Ensures meals are prepared according to Individual Healthcare plans.
  • Provides personal care support to individuals which includes changing, dressing, and other ADL’s.
  • Completes all documentation and notes as required using the Therap system and other electronic systems.
  • Ensures that goals are being completed according to each individual’s ISP.
  • Conducts evacuation drills and records results.
  • Assists in the planning and facilitation of person-centered outings.
  • Completes behavioral tracking logs as required.
  • Follows all agency, State and Federal laws and policies. Uses Policy Stat as needed to access policies.
  • Completes all required training. Uses Relias to either complete training or register for courses.
  • Manages medical emergencies according to the health service plan. Attends doctors’ appointments, transports residents to medical appointments and emergency room as required and requested.
  • Identifies and reports maintenance issues to supervisor or through Net Facilities if available.
  • Reports to work promptly and on time and does not leave work until assigned shift ends or has approval from supervisor. Observes policies/procedures governing requests for annual, sick, or other leave time.
  • Completes Medication Administration and Proxy training. Administers meds to residents and follows all policies related to MAR entries.
  • Tracks vital signs and other health related information for residents.
  • All other duties as assigned.
Required Skills/Abilities:
  • Excellent verbal and written communication skills. Effective communication skills, both verbal and written, are essential for clear instructions and feedback.

  • Excellent time management skills with a proven ability to meet deadlines.
  • Strong analytical and problem‑solving skills.
  • Ability to prioritize tasks and to delegate them when appropriate.
  • Ability to function well in a high‑paced and at times stressful environment.
  • Proficient with Microsoft Office Suite or related software.
  • Ability to use technology to record documentation.
Required Certifications and Licenses:

Must have valid driver’s license, CPR/FA, Mindset

Education and Experience:

1-2 years’ experience working with individuals with disabilities and/or related field preferred but not required. Knowledge and ability to use basic computers and technology.
