Qualifications

RN license in Tennessee; eligible or must obtain Tennessee license within 30 days if license is from another state.
One year of public health nursing experience preferred.
Public Health Certificate required as a condition of continued employment.

Responsibilities

Serve as a nurse case manager for patients diagnosed with TB Infection, TB Disease or suspected TB.
Assist with patient care in hospitals, nursing homes, and long-term care facilities.
Prepare reports and presentations for audits.
Track interjurisdictional (IJ) forms and follow up on referrals to specialists.
Coordinate care for TB patients with MPHD’s medical providers.
Document physical charts and electronic databases (PTBMIS).
Learn and follow TB treatment guidelines and protocols.
Participate in training, conferences, outreach events and screenings.
