Job summary

Coastal Horizons Center, Inc. is seeking a dedicated TASC Care Manager to support justice-involved individuals by providing case management and service coordination within the TASC program.

The role operates primarily in Craven and Pamlico counties with a focus on connecting clients to treatment and recovery services. Qualifications include a Bachelor's degree in a human services field, preferred North Carolina Peer Support Specialist certification, and at least 2 years of relevant experience.

Qualifications

• Bachelor's Degree or higher in a human services or related field.
• Preference for North Carolina Peer Support Specialist certification and/or NC Addiction Specialist Professional Practice Board registration.
• Minimum of 2 years of experience in related field.

Responsibilities

• Administer psychosocial screenings/assessments.
• Coordinate linkage to behavioral health treatment and medical, educational, and vocational services.
• Provide ongoing professional care management services for justice-involved population.
• Collaborate with community partners, justice systems, and service providers.
