The Methodist Home of Kentucky seeks a Targeted Case Manager (TCM) at the Mary Kendall Campus to assess client needs and link resources under Medicaid standards. Ideal candidates hold a bachelor’s degree in a human services field with 1 year of behavioral health experience and complete the required trainings for TCM certification.
The Mending Point Behavioral Health- Mary Kendall Campus is seeking a Targeted Case Manager (TCM) who is highly motivated to make a difference in the community and committed to the well-being of their clients. The TCM will utilize experience and knowledge to assess needs of clients and their families in order to link and coordinate the appropriate resources. They are responsible for managing an assigned caseload and responding appropriately to client needs in compliance with Medicaid standards. This includes but is not limited to: assessing client needs; developing, implementing and reviewing service plans; and working with other community resources in achieving client service needs. Caseloads may include diverse populations with a variety of clinical needs. The Targeted Case Manager is a positive team player that promotes a culture of inclusion and participation.
