Job summary

Honor Community Health in Michigan is seeking a Full Time Targeted Case Manager (Level I) to join our Care Management team. The role provides targeted case management within a Certified Community Behavioral Health Clinic (CCBHC) and helps clients access medical, behavioral health, social, educational, legal, and community services.

The ideal candidate has a bachelor’s degree in social work or related field, at least one year of direct service experience with mental health or substance use

Qualifications

• Bachelor's degree in social work, psychology, nursing, human services, or a related field including state licensures and registration as an RN.
• At least one year of experience in providing direct services to clients with mental health and/or substance use disorder symptoms.
• Experience working for a CCBHC or Federally Qualified Health Center preferred.
• Foundational knowledge of behavioral health systems and community resources.

Responsibilities

• The Full Time Exempt, Level I Targeted Case Manager provides targeted case management services within a Certified Community Behavioral Health Clinic (CCBHC).

• Our standard hours are Mon-Fri, 8:30am-5:00pm.
