Painted Grove Recovery in Kentucky seeks a Targeted Case Manager to coordinate care across inpatient and outpatient services and perform biopsychosocial assessments to support sobriety. The role involves discharge planning and collaboration with community resources within a multidisciplinary team.
The position is full-time, Monday–Friday, 9am–5pm, with a salary around $50,000 plus benefits. A Bachelor’s degree and valid driver’s license are required; prior TCM experience is preferred.
