Job summary

Holly Hill Child & Family Solutions in Highland Heights, KY is seeking a Targeted Case Manager to provide trauma-informed, compliant case management for a caseload of up to 25 referred clients, helping families achieve independence and improved well-being.

This full-time role includes paid training, DailyPay, medical/dental/vision insurance, a 401(k) retirement plan, paid time off, and ongoing professional development with opportunities across multiple service lines.

Qualifications

  • Requires Bachelor’s degree in a mental health field and one year of relevant experience.
  • Must meet Medicaid requirement for Behavioral Health Professional or BHP under supervision.
  • Knowledge of mental health diagnoses and community resources.
  • Excellent written and oral communication skills.
  • Ability to manage time independently and stay flexible.
  • Must drive and have valid driver's license and auto insurance.

Responsibilities

  • Provide Targeted Case Management services compliant with Kentucky regulations.
  • Manage caseload of up to 25 referred clients and meet agency utilization requirements.
  • Complete clinical paperwork within agency timelines.
  • Participate in team meetings, staff meetings, and continuing education.
  • Maintain confidentiality in accordance with HIPAA and policies.

What You’ll Do
  • Actively adhere to the six core principles of trauma informed care and promotes a trauma informed culture.
  • Upholds Holly Hill’s principles and core values
  • Responsible for providing Targeted Case Management Services, compliant with regulations set forth by the state of Kentucky
  • Responsible for the management of a caseload of 25 referred clients, and meeting agency utilization requirements.
  • Responsible for completion of clinical paperwork within agency timelines
  • Responsible for active participation in team meetings, staff meetings, and continuing education courses
  • Adheres to ethical standards of professionalism including maintaining confidentiality of each client and their family, in compliance with HIPAA regulations and agency policies.
  • Prioritizes client care, focusing on delivering quality care for clients
  • Seeks to fulfill Holly Hill’s mission by providing quality services to clients served.
  • Participates in Holly Hill’s Performance Quality Improvement (PQI) activities as assigned

Qualifications

Required:

  • Bachelor’s degree in a mental health field with one year of relevant experience
  • Must meet Medicaid requirement for Behavioral Health Professional (BHP) or BHP under supervision.
  • General knowledge of mental health diagnoses and community resources
  • Requires excellent written and oral communication skills.
  • Must be able to independently manage time and be flexible.
  • Provide treatment services while maintaining clear, professional boundaries.
  • Must be able to drive an automobile; local travel required for this position.
  • Have a current/valid driver’s license.
  • Have current/valid auto insurance.

Preferred:

  • Experience working with children and/or families in a mental health setting is preferred
Physical & Mental Requirements

Requires prolonged sitting, some bending, stooping, and stretching. Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard, copier, telephone, calculator, and other office equipment. Requires normal range of hearing and vision to record, prepare and communicate appropriate reports. Occasionally lifts and carries items weighing up to 35 pounds. Requires some exposure to chemicals and solvents.

Must be able to establish priorities, work independently and to carry out decisions and tasks. Must be able to handle and resolve problems and make effective decisions. Must be able to communicate effectively with others in a professional manner. Requires public contact and good interpersonal skills. Must be mentally adaptable and flexible in dealing with a variety of people.
