Trillium Health Resources is seeking a Tailored Care Management Consultant to join our team. This full-time, exempt role supports the Tailored Care Management provider network, ensuring compliance and quality outcomes across our North Carolina catchment.

Based remotely within our NC counties, the consultant provides technical assistance, monitors performance, develops corrective action plans, and collaborates with stakeholders to improve care delivery and regulatory adherence.

Qualifikationen

• High school diploma or GED with 6 years in medical practices, clinical settings, managed care organizations, provider relations, network development, or provider contracting; experience leading project plans and deliverables.
• Associate's degree with 4 years in related fields and similar experience; lead projects.
• Bachelor's degree with 2 years in related fields and similar experience; lead projects.
• Equivalent combination of education and experience.

What We’re Looking For

Trillium Health Resources has a career opening for a Tailored Care Management Consultant to join our team! The Tailored Care Management Consultant supports the quality, compliance, and performance of the Tailored Care Management provider network. The role provides technical assistance, monitoring, and performance oversight to support provider compliance and continuous improvement in care management service delivery.

On a typical day, you might:

• Monitor provider compliance with Tailored Care Management requirements and standards.
• Provide technical assistance and guidance to provider organizations.
• Support development and implementation of corrective action plans.
• Conduct reviews to assess compliance with regulatory, contractual, and accreditation requirements.
• Collaborate with internal stakeholders to support network performance and quality outcomes.
• Analyze performance data and recommends improvement strategies.

Qualifications

Required:

• High school diploma or GED and six (6) years of experience with medical practices, clinical settings, managed care organizations, provider relations, network development, or provider contracting, including experience leading project plans and overseeing deliverables; OR
• Associate's degree in population health, healthcare administration, business administration, health, psychology, sociology, social work, nursing, or another relevant human service area and four (4) years of experience with medical practices, clinical settings, managed care organizations, provider relations, network development, or provider contracting, including experience leading project plans and overseeing deliverables; OR
• Bachelor's degree in population health, healthcare administration, business administration, health, psychology, sociology, social work, nursing, or another relevant human service area and two (2) years of experience with medical practices, clinical settings, managed care organizations, provider relations, network development, or provider contracting, including experience leading project plans and overseeing deliverables; OR
• Equivalent combination of education and experience.
• Must have a valid driver's license
• Must reside within Trillium's catchment area, which includes the following counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, Warren, Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, Wilson, Carteret, Craven, Duplin, Jones, Lenior, Onslow, Pender, Sampson, Wayne, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, Scotland, Anson, Guilford, Montgomery, Randolph, and Richmond.
• Must be able to travel within catchment as required.
• Must be legally authorized to work in the United States without requiring sponsorship for employment now or in the future (ex: OPT F-1, J-1).

Preferred

• Master’s degree in a field related to population health, healthcare administration, business administration, health, psychology, sociology, social work, nursing, or another relevant human service area.
