Job summary

Alameda Health System is seeking a Nurse Coordinator in Palliative Care to lead identification and assessment of seriously ill patients and coordinate care across hospitals, clinics, and skilled nursing facilities. The role partners with physicians, APNs, social workers, and care management to ensure consistent, compassionate patient care.

You will manage rounds, triage, and staffing while guiding nursing staff in implementing palliative measures and advance care planning.

Qualifications

• Graduate of an accredited Nursing program with a Bachelors Degree in Nursing.
• 1 year of experience working in palliative care, hospice or with the seriously ill population; five (5) years of nursing experience in hospital, clinic, SNF and/or home care in related clinical area (oncology, geriatrics, ICU, neuro).
• Valid license to practice as a Registered Nurse in the State of California.
• CHPN certification by HPNA must be obtained two (2) years from Date of Hire.

Responsibilities

• Lead the identification and comprehensive assessment of seriously ill patients across the healthcare system.
• Ensure appropriateness of palliative care consultation across units and facilities.
• Participate in development of system-wide structures to support palliative care team assignments and manage patient volumes.
• Prepare and manage the morning census and staffing across facilities.
• Attend morning rounds and contribute to patient discussions with palliative care insight.
• Oversee triage criteria to assign patients across the system for immediate needs.
