Job summary

Alameda Health System is seeking a skilled Registered Nurse to join the Palliative Care team at Highland General Hospital. The role involves system-wide coordination of care, front-line nursing leadership, and active participation in rounds and multidisciplinary discussions.

The ideal candidate has a BSN, California RN license, and at least one year of palliative or seriously ill patient experience, with CHPN certification pursued within two years.

Qualifications

• Graduate of an accredited Nursing program with a Bachelors Degree in Nursing
• One year of experience in palliative care, hospice or with seriously ill population; five years of nursing in hospital, clinic, SNF or home care
• California RN license; CHPN certification within 2 years from date of hire

Responsibilities

• Lead assessment of seriously ill patients across the system
• Ensure appropriate palliative care consultations across units
• Participate in system-wide structures to support team assignments and patient flow
• Prepare and manage census and staffing across facilities
• Attend rounds and contribute to patient care discussions
• Coordinate services with nursing, physicians and care management teams
