Job summary

Alameda Health System in Oakland, CA seeks a Systemwide Clinical Palliative Care Nurse Coordinator to lead the assessment and care coordination for seriously ill patients across hospitals, clinics, and skilled nursing facilities. You will work with palliative care physicians, APNs, learners, chaplains, social workers, and nursing staff to implement system-wide practices.

Requirements include a BSN, CA RN license, and 1 year in palliative care with 5 years nursing.

Qualifications

• BSN required or equivalent.
• Current California RN license required.
• CHPN certification within 2 years of hire preferred/required by some sites.

Responsibilities

• Lead identification and comprehensive assessment of seriously ill patients across the healthcare system.
• Ensure appropriate palliative care consultation across units and facilities.
• Coordinate system-wide education and support for advance care planning and palliative care principles.
• Manage resources, triage consults, and monitor nurse staffing related to palliative care.
