Qualifications

• Graduate of an accredited Nursing program.
• Bachelor's degree required.
• California RN license required.

Responsabilités

• Lead the identification and comprehensive assessment of seriously ill patients across the healthcare system.
• Coordinate palliative care across hospitals, clinics, and skilled nursing facilities.
• Attend rounds and contribute to patient discussions with insights on palliative care across units.
