Requirements include a registered nurse license in California, a Bachelor's in Nursing, and CHPN certification within two years.

Qualifications

• Graduate of an accredited Nursing program with a Bachelors Degree in Nursing.
• Minimum five years of nursing experience in hospital/clinic/SNF/home care in related clinical areas.
• One year of experience in palliative care, hospice, or seriously ill population.
• California RN license; CHPN certification within two years of hire.

Responsibilities

• Lead identification and comprehensive assessment of seriously ill patients across the system.
• Coordinate palliative care consultations and triage across units and facilities.
• Attend rounds, support nursing staff, and implement palliative care measures system-wide.
• Document assessments and progress notes; support advance care planning discussions.
• Educate patients, families, and staff about palliative care options and services.
