Job summary

Northwest Community Hospital is seeking a Registered Nurse Breast Oncology Navigator for the Cancer Institute. The role focuses on leading breast navigation across seven Endeavor Health system hospitals and standardizing workflows to reduce time to treatment and fragmentation of care.

The nurse will coordinate care from diagnosis through treatment and surveillance, mentor site navigators, and ensure accreditation readiness while addressing barriers to care such as transportation and language

Qualifications

• Bachelor's degree in Nursing required.
• Master's degree in Nursing preferred.
• Current Illinois RN license required.
• CPR certification (AHA) required.
• OCN or CBCN preferred and must be obtained within 2 years.

Responsibilities

• Serve as system-wide lead for breast oncology navigation and establish program standards.
• Develop standardized breast navigation workflows, care checklists, and patient contact protocols.
• Collaborate with multidisciplinary leads to integrate navigation into oncology care pathways.
• Monitor system-level navigation performance metrics and report on results.
• Lead accreditation activities related to Commission on Cancer standards.
• Receive referrals for newly diagnosed patients and contact them to coordinate initial appointments.
• Coordinate scheduling of oncology appointments, imaging, and surgical consultations.
• Identify barriers to care and facilitate referrals to social work and financial counseling.
• Provide ongoing support to site navigators and participate in navigation huddles.
