Qualifications

  • Bachelor’s degree in social work from an accredited school and one year of related community experience (preferably case management).
  • Bachelor’s degree in human services or equivalent field with two+ years of related community experience.
  • Bachelor’s degree in a non-human services field with two+ years of related experience.
  • An equivalent combination of training and experience accepted.
  • Possession of or ability to obtain a valid NC driver’s license.

Responsibilities

  • Address client needs; develop strategies for delivery of appropriate services to clients.
  • Educate caregivers about CAP/DA waiver for adults.
  • Submit service requests and referrals for CAP/DA participation.
  • Obtain documentation from medical staff to monitor care needs.
  • Assess beneficiaries annually to determine ongoing CAP/DA suitability.
  • Provide case management for CAP/DA beneficiaries.
  • Ensure plans of care are accurate and updated.
  • Monitor health, safety, and wellbeing; document and report risks.
  • Maintain confidential records and prepare required reports.
  • Refer beneficiaries to related agencies as appropriate.
  • Attend scheduled staffings and unit meetings.
  • Participate in trainings as available.
  • Perform other duties including disaster shelter duty and APS on-call.

Social Worker III—Community Alternatives Program (Adult Services)

JOB SUMMARY

This position performs professional social work to individuals and/or families with personal, social, health and economic needs. Work in this class involves providing intensive social work services in serious and complicated cases which require the flexible use of a wide range of social work skills and intervention techniques. This work includes going into the adult’s home, assessing the situation and evaluating their individual needs. Work may involve providing in-depth assessment of individual and family dynamics and needs, assessment, intervention and treatment of patients and/or families with acute to severe medical and/or emotional disorders and mental illnesses, or advance social work intervention, and instruction. Work is distinguished from the Social Worker II classification by greater involvement in sensitive and traumatic situations such as removal of a client from their homes, catastrophic and terminal illness, and/or family dysfunction. Employees supervise and train lower-level social workers, students, and interns. Employees typically report to a Social Work Supervisor II under general supervision. Employees may have combined investigative and treatment roles or may have separate functions depending on the size and organizational structure of the departments of Social Services. An incumbent in this position may be designated “Social Worker I”, “Social Worker II”, “Social Worker III”, or “Social Worker Trainee”.

MAJOR DUTIES
  • Address client needs; develop strategies for delivery of appropriate services to clients.
  • Educate the caregiver of children, elderly and disabled adult community about the CAP/DA waiver.
  • Submit the service request and referral for individuals seeking to participate in the CAP/DA Waiver.
  • Provide help in obtaining documentation from medical staff to monitor ongoing care needs.
  • Access a beneficiary on an annual basis to determine ongoing appropriateness for CAP/DA services.
  • Provide case management to the CAP/DA waiver beneficiary.
  • Ensure waiver and non-waiver services are accurately listed on the plan of care initially, annually and during a plan of care revision.
  • Provide regular and routine beneficiary monitoring to assure health, safety, and wellbeing, quality assurance reporting and beneficiary risk mitigation.
  • Maintain confidential records and files and prepare reports as required.
  • Assist and support clients in their use of agency and other resources.
  • Refer beneficiaries to related agencies as appropriate.
  • Attend scheduled staffing with supervisor and unit meetings
  • Attend training sessions as available.
  • Perform other related duties as assigned, including disaster shelters and possible APS on-call.
KNOWLEDGE REQUIRED BY THE POSITION
  • Knowledge of methods and principles of casework and training.
  • Knowledge of social work principles, techniques, and practices and their application to specific casework and community problems.
  • Knowledge of behavioral and socioeconomic problems and their treatment and governmental and private organizations and community resources.
  • Knowledge of the laws, regulations, and policies which govern social work programs.
  • Skill in expressing ideas clearly and concisely and in planning and executing work effectively.
SUPERVISORY CONTROLS
  • The Social Work Supervisor II assigns work in terms of very general instructions. The supervisor spot-checks completed work for compliance with procedures and the nature and propriety of the results.
GUIDELINES
  • Guidelines include social work principles and methods, North Carolina General Statutes, federal, state, and county policies and procedures, court guidelines, the Social Worker Code of Ethics, and confidentiality guidelines. These guidelines require judgment, selection, and interpretation in application.
COMPLEXITY
  • The work consists of varied duties in the delivery of assistance to agency clients. Strict regulations and the volume of cases contribute to the complexity of the position.
SCOPE AND EFFECT
  • The purpose of this position is to provide assistance to agency clients. Successful performance results in the provision of social services to clients.
PERSONAL CONTACTS
  • Contacts are typically with co-workers, other county employees, health care providers, law enforcement personnel, court officials, representatives of community groups, school personnel, clients, and members of the public.
PURPOSE OF CONTACTS
  • Contacts are typically to give or exchange information, provide services, motivate people, and resolve problems that involve the safety and well-being of the elderly, and disabled adults.
PHYSICAL DEMANDS
  • The work is typically performed while sitting at a desk or table or while intermittently sitting, standing, or stooping. The employee occasionally lifts light objects.
WORK ENVIRONMENT
  • The work is typically performed in an office and outdoors, occasionally in cold or inclement weather. The employee may be exposed to infectious diseases. The work could require the use of protective devices such as masks or gloves.
SUPERVISORY AND MANAGEMENT RESPONSIBILITY

MINIMUM QUALIFICATIONS
  • Bachelor’s degree in social work from an accredited school of social work, and one (1) year of directly related community experience (preferably case management) in the health or medical field directly related to homecare, long-term care, or personal care.
  • Bachelor’s degree in human services or equivalent field from an accredited college or university with two or more years of community experience (preferably case management) in the health or medical field directly related to homecare, long-term care, or personal care.
  • Bachelor’s degree in a non-human services field from an accredited college or university with two or more years of community experience (preferably case management) in the health or medical field directly related to homecare, long-term care, or personal care.
  • An equivalent combination of training and experience.
  • Possession of or ability to readily obtain a valid driver’s license issued by the State of North Carolina for the type of vehicle or equipment operated.
