Senior Suites at Century Farms is seeking a Survey Readiness & Quality Assurance Nurse to support two Ohio assisted living communities. You will ensure regulatory compliance with ODH, maintain survey readiness, and lead quality improvement initiatives across both locations.

The role requires organization, audit oversight, and staff education, with responsibility to prepare for state surveys and develop corrective action plans as needed.

Qualifikationen

• Active Ohio RN or LPN license in good standing.
• Minimum 3 years of experience in Assisted Living / Long-Term Care or similar.
• Experience in survey preparation, QA, compliance, and corrective actions.

Aufgaben

• Float between Century Farms Assisted Living and St. Clair Commons Assisted Living as needed.
• Maintain ongoing survey readiness in line with ODH regulations.
• Conduct audits of resident care plans, medication administration, and clinical documentation.
• Lead Plans of Correction (POCs) and monitor corrective actions.
• Provide staff education on ODH regulations, documentation standards, and infection control.
