Job summary

The position supports patient discharge planning and care coordination within our hospital system. Qualified candidates must be licensed as an RN in the state, have or pursue a Bachelor’s in Nursing, and hold current BLS certification.

Qualifications

• Graduate from an accredited and approved school of nursing accepted for licensure in the state of practice is required.
• Bachelor’s degree in Nursing is preferred.
• Licensed as a Registered Nurse (RN) in state of practice is required.
• Basic Life Support (BLS) is required.
