Responsibilities

• Assess: Performs assessment of physical, social, and psychological status and gathers information from the care team.
• Plan: Develops individualized plan of care with patient and family involvement.
• Implement: Coordinates care, delegates tasks, manages time and resources.
• Evaluate: Reassesses and revises the plan based on outcomes and satisfaction.
• Professionalism: Adheres to legal/ethical standards, documents accurately, and maintains licensure.
