Responsibilities

• Completes nursing assessment of patient and documents in patient record.
• Develops a plan of care based on patient assessment and age/developmental needs.
• Collaborates with physicians and health team in developing the plan of care.
• Delivers, delegates, and coordinates the plan of care.
• Initiates and participates in discharge planning and patient/family teaching.
• Evaluates patient responses to care and updates plan as needed.
• Participates in performance improvement activities and safety/compliance.
