Position Overview

The Supportive Living Companion works under immediate supervision, providing tailored services and activities within the consumer's home and community. The role involves ensuring services are delivered as designed to help consumers acquire, retain, and improve self-help, socialization, and adaptive skills aligned with active treatment goals. Responsibilities include incident reporting, completing activity logs linked to person-centered plans, maintaining required documentation, and ensuring compliance with personnel policies.

Work Schedule

Monday-Friday, 12:00 PM - 8:00 PM

Essential Duties and Responsibilities

1. Provide services and activities per the consumer’s person-centered plan.
2. Support independent living skills, home management, cooking, cleaning, socialization, and supported employment.
3. Report incidents within 24 hours according to DHS policy.
4. Complete activity logs as per plan objectives.
5. Report any changes affecting the individual.
6. Remind consumers about medication management if necessary.
7. Complete required documentation.
8. Meet ongoing training requirements.
9. Maintain compliance with personnel documents.
10. Perform other duties as assigned.

Qualifications

• High School diploma or GED.
• At least 1 year of relevant experience in human services or 2 years working with individuals with developmental disabilities.
• CNA or Medical Assistant certification qualifies for a special rate.
• Ability to understand written activity plans, communicate effectively, access emergency and transportation services, pass background checks, and complete drug screening.

Additional Requirements

• Must pass background checks and drug screen.

Compensation and Employment Details

Salary: $12.00 - $12.50 per hour
