General Description Of Position

The Supportive Living Companion works under immediate supervision, providing tailored services and activities within the consumer's home and community. They ensure services are delivered as designed to help consumers acquire, retain, and improve self-help, socialization, and adaptive skills, aligning with active treatment goals and objectives. Responsibilities include reporting incidents, completing activity logs linked to person-centered plans, providing required documentation, and maintaining compliance with personnel policies.

Schedule: TUESDAY-FRIDAY, 5:00PM-10:00PM (11520/BP)

Essential Duties and Responsibilities

1. Provide services and activities to consumers in their home and community based on their person-centered plan goals.
2. Support individuals in developing independent living skills, home management, cooking, cleaning, socialization, supported employment, and other skills to foster independence.
3. Follow procedures within 24 hours for incident reporting as per Department of Human Services policy.
4. Complete activity logs during work hours, aligned with person-centered plan objectives.
5. Report any changes affecting the individual receiving services.
6. Remind consumers about medication refills if necessary.
7. Complete and submit required documentation.
8. Meet initial and ongoing training requirements.
9. Maintain compliance with personnel documentation.
10. Perform other related duties as assigned.

Education and Experience

• High School diploma or GED.
• One year of relevant supervised experience in public health, human services, or community agencies, OR two years of successful experience working with individuals with developmental disabilities.
• CNA or Medical Assistant certification qualifies for a special entry rate.
• Must understand written activity plans, communicate effectively, access emergency services and transportation, and pass background and maltreatment checks every 2-5 years.
• Must pass a pre-employment drug screen.

Salary: $12.00 - $12.50 per hour

Additional Information: Must be able to pass background checks and drug screening. Employment offers are contingent upon these results.
